As a board-certified physician in both Family and Sports medicine, Dr. Mottl is highly qualified to manage a wide spectrum of medical conditions including routine illnesses, general medical care, and acute injuries.

Payments with AHCCCS, Medicaid or Medicare are not accepted at this time.
Certain age restrictions may apply if you qualify for any of the above-mentioned plans, such as Medicare.
This is not a “Walk-in” Urgent Care services practice. Scheduling for appts. is required.
My practice DOES NOT provide any form of Emergency care service.

Evaluation and treatment of any single concern:

Evaluation of Intake/Medical history forms.
Face-to-face time with a medical doctor for evaluation of a single health concern.
Discussion of treatment options and alternatives, and/or medical counseling as needed.

Preventive Visit

Routine health exams, including:
Monitoring of acute and chronic conditions, as well as preventive/health-promoting counseling.
Ordering of basic labs (CMP and Lipid panel).
Review of medications and ordering of refills, if needed (I do not prescribe medications for chronic pain}
Sports/Pre-employment Physicals
Includes review and signing of forms and/or "Statements of Good Health", as indicated.

Evaluation of Intake/Medical forms if any.
Face-to-face time with a medical doctor for evaluation and management of a specific health concerns.
Discussion of treatment options and alternatives, and/or medical counseling if needed.
Essential lab-work for a Preventive Physical Exam (CMP and Lipid panel), if required.
Preparation and certification of health forms only as above (if required).

*Basic labs: Comprehensive metabolic panel and a Lipid panel.
*Medications and other lab orders accounted separately, if needed.
Appointment details
Standard visit - 20 minutes consultation
Preventive visit - 30 minutes consultation
- Our practice does not provide walk-in urgent care services.
- Hours of operation are Monday through Thursday, 5:30 pm to 8:00 pm.
- Same-day appointments must be scheduled no later than 4:00 pm that afternoon.
- The last appointment of the day is 7:40 pm at night.
- Wellness visits and pre-participation physical exams will not be scheduled for same day appointments and must be scheduled at least one day in advance.
- In an emergency situation or anything that could possibly be perceived as an emergency situation, Patients should proceed to the nearest emergency room or call 911.
Exclusive approach
- This practice does not take any form of health insurance to pay for visits and/or any services provided.
- At this time, our practice cannot extend our services to recipients of Medicare/Medicaid benefits(or any individual age 62 or older), VA, Tricare, or any recipient or Federal funding for health services.
- We do not offer Federal Motor Carrier Safety Administration (“FMCSA”) physicals, disability determinations for insurance, social security, or ADA purposes; and/or Workman’s Compensation visits.
- Required lab work and/or imaging will be ordered by our Practice and requires our patients to go to an outside facility for the sample collection (or study) to take place.
House calls
$195 for appointment
- House calls are at the physicians’ sole discretion and must be scheduled through the portal (via the link below) at least four hours in advance and are subject to the physician’s availability.
- Area of service bounded as follows: East of I-10, South of Tangerine road, West of N Sabino Canyon Rd., and North of Grant rd.
- Services offered for house calls are the same as for all office appointments.

Most healthcare providers, including myself, are required to bill Medicare for services provided to patients who qualify for Medicare benefits (typically age 62 and older). Because our practice does not bill to any form of insurance, we are unable to offer services to Medicare-eligible patients. However, we can assist in guiding you to similar care options available under a fee-for-service structure for those with Medicare coverage.

A typical medical office visit comprises several essential components, each tailored to the patient's specific needs and the visit's purpose. These include the measurement of vital signs, evaluation for a specific health concern, diagnosis and treatment planning, patient education and counseling, as well as any necessary follow-up or referrals.

House call visits includes the same components as a regular medical office visit, with the exception that the encounter takes place at the patient's home.
